Текст оригинала - английский Options for the repair of parastomal hernias include contralateral transposition or in situ repair. The latter can be accomplished either primarily or with prosthetic mesh. Concerns with mesh include possible gut erosion and infection. Recurrence rates in the literature are dismal regardless of technique. We retrospectively reviewed our experience with this problem focusing on in situ repairs. We identified 9 patients who underwent 10 in situ repairs. Of these, 6 were women, average age was 69.4 years, and stomas had been constructed for cancer in 6, inflammatory bowel disease in 2, and incontinence in 1. Eight patients had colostomies; one had an ileostomy. All patients were symptomatic from their hernias. Repairs were performed an average of 8 years after stoma construction. Hernia repair was performed transabdominally in four and through a parastomal incision in six. Complications included hematoma formation requiring evacuation in one and delayed resumption of oral intake secondary to nausea and cramps in three. Of the 9 initial repairs, 1 recurred (11%) and was repaired without subsequent failure. No mesh erosions or wound infections have occurred. This technique is safe and may be preferable to contralateral placement of the stoma.

Ми здійснили ретроспективний огляд власного досвіду вирішення цієї проблеми, зосередивши увагу на випадках герніопластики in situ. Нами було обрано 9 пацієнтів, які перенесли 10 операцій in situ. Шість із них – жінки; середній вік їх становив 69,4 років; шести з них було накладено стому в ході лікування від раку, двом – від запальних захворювань кишечника, і ще одному – від нетримання. Вісім пацієнтів мали колостому, а ще один – ілеостому. У всіх пацієнтів спостерігалися клінічні прояви грижі. Герніопластика проводилася в середньому через 8 років після накладання стоми. Чотирьом пацієнтам було здійснено герніопластику із очеревинним доступом, і ще шістьом – через парастомічний розріз. Серед ускладнень спостерігалося утворення гематоми, що потребувала оперативного видалення, в одного з пацієнтів, і сповільнене відновлення прийому їжі, що супроводжувалося нудотою та спазмами, – ще у трьох. Із дев'яти пацієнтів, що вперше перенесли герніопластику, рецидив трапився лише в одного (11 %), і повторна операція виявилася успішною. Recommendations:
• Persons with COVID-19 who are prescribed angiotensin-converting enzyme (ACE) inhibitors or angiotensin receptor blockers (ARBs) for cardiovascular disease (or other indications) should continue these medications (AIII).
• The COVID-19 Treatment Guidelines Panel (the Panel) recommends against the use of ACE inhibitors or ARBs for the treatment of COVID-19 outside of the setting of a clinical trial (AIII).
Angiotensin-converting enzyme 2 (ACE2) is the cell surface receptor for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). It has been hypothesized that the modulation of ACE2 associated with these therapies could suppress or enhance SARS-CoV-2 replication. Investigations of the role of ARBs and recombinant human ACE2 in treatment and prevention of SARS-CoV-2 infection are underway.
Whether these medications are helpful, harmful, or neutral in the pathogenesis of SARS-CoV-2 infection is unclear. Currently, there is a lack of sufficient clinical evidence demonstrating that ACE inhibitors or ARBs have any impact on the susceptibility of individuals to SARS-CoV-2 or on the severity or outcomes of infection. This recommendation is in accord with a joint statement of the American Heart Association, the Heart Failure Society of America, and the American College of Cardiology.
Corticosteroids
Systemic corticosteroids can affect the pathogenesis of viral infections in various ways. In outbreaks of other novel coronavirus infections (i.e., Middle East respiratory syndrome [MERS] and severe acute respiratory syndrome [SARS]), corticosteroid therapy was associated with delayed virus clearance. In severe pneumonia caused by influenza, corticosteroid therapy may worsen clinical outcomes, including secondary bacterial infection and mortality. Conversely, the potent anti-inflammatory effects of corticosteroids are proposed to have a potential therapeutic role in suppressing cytokine-related lung injury. Data on the use of corticosteroids in COVID-19 are limited. The recommendations for use of corticosteroids in patients with COVID-19 depend on the severity of illness, indication, and underlying medical conditions and should be considered on a case-by-case basis.

С другой стороны, предполагается, что мощное противовоспалительное действие кортикостероидов потенциально способно играть терапевтическую роль в подавлении связанного с цитокинами повреждения легких. Данные о применении кортикостероидов при COVID-19 ограничены. Заінтересована особа звертається до центрального органу виконавчої влади у сфері охорони здоров'я (далі за текстом — «МОЗ України») з обґрунтованим клопотанням про надання Кабінетом Міністрів України дозволу на використання запатентованого виноходу (корисної моделі), іншого об'єкта права інтелектуальної власності, що стосується лікарського засобу (далі за текстом — «об'єкт»).
В клопотанні зазначаються міжнародна непатентована назва лікарського засобу, найменування об'єкта, номер відповідного патенту (свідоцтва), відомості про його власника, його адреса (або місцезнаходження), а також подається — документальне підтвердження надання власником патенту безпідставної відмови у видачі ліцензії на використання об'єкта.

2. The authorization shall be granted for the purpose of ensuring public health protection, environmental safety, and satisfaction of other public interests, including cases of natural disasters, catastrophes, epidemic diseases, etc.
The authorization may be granted to any person who, under the current legislation of Ukraine, may be a holder of a marketing authorization for a medicinal product (obtained by the applicant) and has expressed an intention to use a patented invention (utility model), or another item of intellectual property regarding a medicinal product, for the purpose of its application in Ukraine (hereinafter referred to as interested person).
3. The interested person shall apply to the central executive authority for health care (hereinafter referred to as the Ministry of Health of Ukraine) with a reasonable request for the Cabinet of Ministers of Ukraine to grant authorization to use a patented invention (utility model) or another item of intellectual property regarding a medicinal product (hereinafter referred to as item).
The request shall include the International Nonproprietary Name of the medicinal product, the item’s name, the number of the relevant patent (authorization), the information on its holder, and their address (or location); a documentary proof of the patent holder’s unjustified refusal to issue a license to use the item shall also be submitted.

The axillary lymph nodes on the right are not enlarged.
The supra- and subclavicular lymph nodes on the right and on the left are not enlarged.
On the left, there are changed axillary lymph nodes under the pectoralis minor, with contrast accumulation, 2 × 3 mm to 7 × 18 mm in size, N.10.
On the border of the upper quadrants of the left breast, there is a hypovascular lesion without sharp contours, 8 × 7 mm in size. In the right breast, there are no nodular lesions.
In the pleural cavities on both sides and the pericardial cavity, no effusion is visible. No bone destructive changes are visible at this level. There are degenerative changes in the thoracic spine.
Conclusion:
CT signs of left breast cancer.

In Zusammenarbeit zwischen der Landeshauptstadt (Stelle für internationale Angelegenheiten und Kulturreferat), der Hanns-Seidel-Stiftung, Misereor und der Harare-München-Partnerschaft (HaMuPa), ist eine Konferenz zum Thema “Politische Bürgerbeteiligung und bürgerschaftliches Engagement in Fragen der Stadtentwicklung (und Staatsentwicklung) geplant.
Vor dem Hintergrund der jüngsten politischen Entwicklungen in der Ukraine und in Zimbabwe soll der Fokus der Veranstaltung vor allem auf das Engagement und die Situation der zivilgesellschaftlichen Akteure in Münchens Partnerstädten Kiew und Harare gerichtet werden, aber auch das bürgerschaftliche Engagement in München darstellen.
Allen beteiligten Organisationen soll die Möglichkeit geboten werden, von den Erfahrungen der jeweils anderen Partnerorganisationen zu lernen.
Aufgrund der Zusammensetzung der Akteure (München-Kiew-Harare) ist zu erwarten, dass sich dabei verschiedene Horizonte ergeben: Für die Teilnehmen aus München, wo zivilgesellschaftliches Engagement eher durch die Verdrossenheit des Einzelnen als durch staatliche Einschränkungen leidet, ist die Erfahrungswelt sicher eine andere, als für die Teilnehmer aus Harare, wo zivilgesellschaftliches Engagement vom Staat als Bedrohung angesehen wird, und für die Akteure durch das dortige Gewaltregime äußerst gefährlich ist. Zivilgesellschaftliches Engagement dient dort jedoch, in Ermangelung eines funktionierenden Mehrparteiensystems, verstärkt als politisches Kontrollinstrument. Teilnehmer/Innen aus Kiew können in diesem Szenario eine wichtige Mittlerrolle einnehmen, da sie in ihrer Geschichte die Phase der Zivilgesellschaft, wie sie derzeit in Harare besteht, bereits überwunden haben. In Kiew haben sich zivilgesellschaftliche Akteure erfolgreich gegen einen repressiven Staat durchgesetzt und streben nun eine Weiterentwicklung nach europäischem Vorbild an.
Konkret ist geplant, Vertreter von Bürgerorganisationen sowie einzelne Kommunalpolitiker aus München, Harare und Kiew (hier je ca. 7-8 Teilnehmer unterschiedlichster Bürgerbewegungen) einzuladen und so einen Dialog mit Vertretern von zivilgesellschaftlichen Organisationen in München zu ermöglichen. Insgesamt soll die Konferenz für ca. 80 Personen ausgerichtet werden.

У співпраці між Службою з міжнародних питань та управлінням культури столиці федеральної землі Мюнхена, фондом Ханса Зайделя, благодійною організацією «Мізереор» та асоціацією «Партнерство Хараре-Мюнхен» (HaMuPa) було заплановано конференцію з теми «Участь громадян у політичних процесах і діяльність на громадських засадах з метою вирішення питань розвитку міста та держави».
З огляду на останні політичні події в Україні та Зімбабве центром уваги на конференції має стати передусім діяльність на громадських засадах у містах-побратимах Мюнхена, Києві та Хараре, а також діяльність громадських організацій Мюнхена.
Усім організаціям, що беруть участь у конференції, має бути надана можливість переймати досвід інших організацій-партнерів у вирішенні тих чи інших проблем.
Склад учасників конференції (Мюнхен, Київ і Хараре) дає підстави сподіватися на відкриття нових горизонтів: адже учасники з Мюнхена, де діяльність громадських організацій зазнає проблем радше в одиничних випадках, а не через систематичні обмеження з боку держави, мають абсолютно інший досвід, ніж учасники з Хараре, де діяльність на громадських засадах сприймається державою як загроза, і таким чином становить надзвичайну небезпеку для громадських діячів з боку тоталітарного режиму їхньої країни. В той же час у Хараре діяльність на громадських засадах, в умовах відсутності функціонуючої багатопартійної системи, виступає надзвичайно потужним інструментом політичного контролю. Учасники з Києва можуть, що надзвичайно важливо, виступити як посередники у цій ситуації, адже в історії їхньої країни вже перейдений той етап формування громадянського суспільства, на якому наразі перебуває Хараре. У Києві громадські діячі вийшли переможцями з боротьби проти репресій із боку держави, і нині вони прагнуть до подальшого розвитку з метою наближення до європейських стандартів.
Серед конкретних планів - запрошення представників громадських організацій, а також діячів органів місцевого самоврядування з Мюнхена, Хараре та Києва (приблизно по 7-8 учасників найрізноманітніших громадських рухів), та налагодження діалогу з представниками громадських організацій у Мюнхені. Загалом конференція розрахована приблизно на 80 учасників.

1. Was ist Verfahrenshilfe?
Verfahrenshilfe befreit die Parteien eines Gerichtsverfahrens nur vorläufig von der Pflicht zur Entrichtung der eigenen Prozesskosten.
ACHTUNG:
Sollten Sie den Prozess verlieren, müssen Sie die Prozesskosten der gegnerischen Partei (hierzu zählen insbesondere deren Gerichts- und Anwaltskosten) trotz der Ihnen gewährten Verfahrenshilfe ersetzen.
Sofern Ihnen Verfahrenshilfe gewährt wurde, kann Sie das Gericht innerhalb von drei Jahren nach Abschluss des Verfahrens zur gänzlichen oder teilweisen Nachzahlung der gewährten Vergünstigungen verpflichten, soweit und sobald Sie ohne Beeinträchtigung des notwendigen Unterhalts dazu imstande sind. Diese Nachzahlungsverpflichtung kann auch dann auferlegt werden, wenn Sie den Rechtsstreit gewinnen und dadurch zu ausreichendem Vermögen gelangen. Davon umfasst ist die tarifmäßige Entlohnung des Ihnen beigegebenen Rechtsanwalts.
2. Was sind die Voraussetzungen für die Bewilligung?
Verfahrenshilfe ist vom Gericht nur zu bewilligen, wenn das konkrete Verfahren zu einer Beeinträchtigung des notwendigen Unterhalts führen würde. Notwendiger Unterhalt ist jener, den Sie für sich und ihre Familie, für deren Unterhalt Sie zu sorgen haben, zu einer einfachen Lebensführung benötigen.
Damit die Verfahrenshilfe gewährt wird, darf die beabsichtigte Rechtsverfolgung oder Rechtsverteidigung nicht offenbar mutwillig oder aussichtslos sein.
3. In welchem Umfang wird Verfahrenshilfe gewährt?
Verfahrenshilfe ist nur soweit zu gewähren, als sie absolut notwendig ist. Deshalb kann das Gericht Sie z.B. auch zu einer Ratenzahlung verpflichten. Auch die Vertretung durch einen Rechtsanwalt kann nicht in jedem Fall gewährt werden. Nur wenn die gesamte Kostenlast unzumutbar ist, kann die Verfahrenshilfe im vollen Ausmaß bewilligt werden. Bei Bewilligung der Verfahrenshilfe wird ausgesprochen, welche der aufgezählten Begünstigungen zur Gänze oder zum Teil gewährt werden.
4. Wo beantrage ich die Verfahrenshilfe?
Der Antrag auf Verfahrenshilfe ist mündlich oder schriftlich bei dem Gericht einzubringen, bei dem das Verfahren geführt wird oder werden soll. Befindet sich der Sitz dieses Gerichts außerhalb des Bezirksgerichtssprengels, in dem Sie Ihren Aufenthalt haben, so können Sie den Antrag auch beim Bezirksgericht Ihres Aufenthaltes zu Protokoll erklären.

1. Что такое материальная помощь для покрытия судебных издержек?
Материальная помощь для покрытия судебных издержек лишь временно освобождает стороны судебного процесса от обязательства по оплате собственных судебных издержек.
ВНИМАНИЕ:
Если Вы проиграете процесс, то должны будете возместить противоположной стороне судебные издержки (к ним относятся, в частности, судебные издержки и расходы на оплату услуг адвоката), несмотря на предоставленную Вам материальную помощь для покрытия судебных издержек.
Если Вам была оказана материальная помощь для покрытия судебных издержек, суд может в течение трех лет после окончания производства по делу обязать Вас полностью или частично оплатить задним числом предоставленные льготы, насколько и как только Вы будете способны это сделать без ущерба необходимым средствам к существованию. Это обязательство по оплате задним числом может также быть возложено в случае, если Вы выиграете судебный процесс и таким образом в достаточной степени обезопасите себя. Это касается также и выплаты вознаграждения предоставленному Вам адвокату согласно тарифу.
2. Каковы условия для получения разрешения?
Разрешение на получение материальной помощи для покрытия судебных издержек предоставляется судом только в случае, если конкретный судебный процесс может нанести ущерб необходимым средствам к существованию. Необходимые средства к существованию — те, в которых Вы нуждаетесь для обеспечения себе и своей семье, о поддержке которой Вы должны заботиться, удовлетворительного образа жизни.
Материальная помощь для покрытия судебных издержек может быть обеспечена при условии, что запланированный судебный иск или защита не является откровенно злонамеренным или бесперспективным.
3. В каком объеме оказывается материальная помощь для покрытия судебных издержек?
Материальная помощь для покрытия судебных издержек предоставляется только при условии, что она совершенно необходима. Поэтому суд может обязать Вас, например, к оплате в рассрочку. Также не всегда может быть обеспечено представительство адвокатом. Только при условии, что общее бремя судебных издержек является непосильным, материальная помощь для покрытия судебных издержек может быть разрешена в полном объеме. При разрешении материальной помощи для покрытия судебных издержек указывается, какие из перечисленных льгот предоставляются полностью или частично.
4. Куда следует подавать заявление на получение материальной помощи для покрытия судебных издержек?
Заявление на получение материальной помощи для покрытия судебных издержек должно быть подано в суд, где осуществляется или должно осуществляться производство по делу, в устной или письменной форме. Если этот суд находится за пределами участка окружного суда, на территории которого Вы проживаете, то Вы также можете подать заявление для занесения в протокол в окружном суде по месту Вашего жительства.

I am a graduate of Kyiv National Linguistic University, holding a Master’s degree in Translation and Interpreting from English and German.
Before starting my career as a freelancer, I had several years of experience as both an in-house translator and a project manager at translation agencies, which has given me a lot of valuable insights into the administrative side of the translation industry as well as a much deeper understanding of the end clients’ needs.
My primary area of specialization is pharmaceutical and medical translation. I have translated a vast volume of documentation for major pharmaceutical companies, contract research organizations, medical device manufacturers, and healthcare agencies, including but not limited to the following types of documents:
clinical trial materials (case report forms, clinical study protocols, clinical study reports, clinical trial agreements, curricula vitae of investigators, drug labels, institutional review board (IRB)/independent ethics committee (IEC) opinions, investigator’s brochures, patient diaries, patient information sheets and informed consent forms, patient questionnaires, patient reported outcomes, recruitment materials, regulatory authority approvals);
pharmaceutical industry documents (analytical procedures, certificates of analysis, certificates of pharmaceutical product, drug master files, GMP certificates, manufacturing and import authorizations, patient information leaflets, periodic safety update reports, regulatory authority correspondence, summary of product characteristics, standard operating procedures, validation reports);
medical device manuals (diagnostic and surgical tools, laboratory equipment);
medical records (case records, diagnostic findings, hospitalization summaries, medical certificates);
public health materials (brochures, flyers, guides, leaflets, posters).
Another area in which I have substantial experience is legal and business translation. I have dealt with numerous projects involving translation and notarization of documents such as agreements, contracts, articles of association, minutes of shareholders’ meetings, affidavits, powers of attorney, memorandums, statutes, court papers, business letters, bid documents, financial statements, company policies, and promotional materials; I have also worked a lot with personal documents such as passports, IDs, certificates, diplomas, references, CVs, etc.
I believe in using CAT technology whenever possible, leveraging translation memories and glossaries tailored to individual client needs in order to deliver translations with consistent terminology and industry-specific wording that can be used efficiently for their intended purpose. Moreover, I am a highly detail-oriented linguist when it comes to quality assurance, and in proofreading both other translators’ and my own work I always do my best to eliminate any stylistic flaws, ambiguities, or errors so that the final text reads smoothly and conveys the source meaning precisely.
I am committed to continuing professional development and do my best to strengthen and enhance my knowledge in my working subject fields by reading industry-specific literature and taking online courses on platforms such as Coursera, edX, Prometheus, etc. (for more information, see Certificates tab on my profile).
