This is the title of a one-page testimonial, a description of events witnessed by the author.
Would it be translated as simply "testimonial"?

The terms "deed of testimony," "testimonial deed," etc. don't seem to exist.
I've seen "testimony deed" used one time, but I'm not sure if one time is enough to use that term.
