English Medical Medical: Health Care Patient Report
This abbreviation (I assume it is) is on a patient report for a patient examined by a consultant (an orthopedist) after a road accident.
After the description of X-ray results and some minor injuries on hand-arm it is located as below:

XR R hand: NBI (no bone injury)

d/c

Dr. XXX

Can you please let me know what does d/c stands here?

agree Peter Simon : Also at http://medical-dictionary.thefreedictionary.com/D/C. It could also be understood as 'discharged', referring to the fact that the patient has been discharged from his care.
